Plantar fasciitis is the most common cause of heel pain, and it is one of the conditions we treat most often across our Fort Lauderdale and Plantation offices. The plantar fascia is a strong, sheet-like band of connective tissue that spans the sole of your foot, connecting your heel bone (the calcaneus) to the base of your toes. It works like a shock absorber and a spring, supporting your arch every time you take a step.
When that tissue is loaded faster or harder than it can handle, small areas near where it attaches to the heel become irritated. The name ends in “-itis,” which suggests pure inflammation, but current understanding describes plantar fasciitis as more of a load-related, degenerative change in the fascia than a simple inflammatory flare. That distinction matters, because it explains why rest alone often is not enough and why rebuilding the tissue’s capacity is such an important part of lasting relief.
It helps to think of heel pain in two stages. Acute plantar fasciitis is recent, reactive, and often very sensitive, with that hallmark first-step pain. Persistent or recalcitrant plantar fasciitis has been lingering for months and tends to need a more structured, progressive plan. Both respond well to conservative care in the large majority of cases, and surgery is rarely required.
Two myths:
One: A heel spur is the cause of the pain. Heel spurs are common, frequently painless, and often show up in people with no symptoms at all, so a spur on an X-ray does not confirm that it is the source of your heel pain.
Two: Resting until it goes away is the best strategy. Complete rest can calm symptoms briefly, but without addressing footwear, calf flexibility, and gradual loading, the pain commonly returns the moment activity resumes.
How Plantar Fasciitis Feels Day to Day
Most people describe a sharp, stabbing, or bruise-like pain near the inside of the heel, sometimes spreading into the arch. The single most recognizable pattern is pain with the first few steps in the morning, which is why so many of our patients arrive searching for why their heel hurts when they get out of bed. The same first-step sensation often returns after sitting for a while or after getting up from rest.
The pattern of what makes it better and worse is just as telling as the pain itself. Many people notice that the bottom of the foot hurts near the heel when they first stand, then loosens up after a few minutes of walking, only to ache again after a long day. Foot arch pain after standing all day is a frequent complaint, and prolonged walking, climbing stairs, or going barefoot on hard floors can aggravate it. Heel pain that will not go away despite weeks of trying to ignore it is one of the most common reasons people finally book an evaluation.
Day to day, plantar fasciitis reaches beyond the foot. It can disrupt sleep when the heel throbs after an active day, make a standing job genuinely uncomfortable, and quietly shrink your exercise routine as you avoid running, walking, or anything that loads the foot. Left unaddressed, people often change how they walk to protect the heel, which can create new aches in the knee, hip, or lower back.
Why Plantar Fasciitis Happens (The Common Drivers)
A fair question we hear often is, why me? Plantar fasciitis usually is not caused by any single thing. It develops when the demand placed on the fascia outpaces what it is currently prepared to handle, and several factors tend to stack up together.
Mechanical and load factors are the most direct contributors. Tight calf muscles, the gastrocnemius and soleus, pull on the heel and increase strain through the fascia. Foot mechanics play a role too, since both flat feet with overpronation and very high, rigid arches can change how force is distributed across the sole. A sudden spike in activity, such as ramping up running mileage, starting a new sport, or a job that suddenly has you on your feet, is a classic trigger.
Lifestyle and everyday factors add to the picture. Long hours standing or walking on hard surfaces, unsupportive shoes, worn-out sneakers, and going barefoot on tile floors all raise the daily load. Carrying extra body weight increases the force the heel absorbs with every step, which research consistently links to a higher risk of plantar heel pain.
Underneath all of it is a simple mismatch between capacity and recovery. When training, standing time, or step count climbs faster than the tissue can adapt, and when sleep, footwear, and recovery do not keep up, the fascia stays irritated. The encouraging part is that most of these drivers can be adjusted, which is exactly where a tailored plan comes in.
Red Flags: When Heel Pain Needs Urgent Attention
Plantar fasciitis is rarely an emergency, but heel and foot pain can occasionally signal something that needs prompt medical evaluation rather than conservative care. Please seek urgent or emergency attention, or contact your physician promptly, if you notice any of the following:
- Sudden, severe heel or foot pain immediately after a fall, twist, or impact, especially if you cannot bear weight, which can indicate a fracture.
- Numbness, tingling, burning, or a pins-and-needles feeling spreading into the foot or toes, which may point to nerve involvement rather than the fascia.
- Fever, redness, warmth, or swelling around the heel, which can suggest infection.
- Heel pain accompanied by significant calf swelling, which warrants ruling out a blood clot.
- Night pain that wakes you from sleep, unexplained weight loss, or pain that is steadily worsening despite rest, which deserves a closer medical workup.
- A history of diabetes or poor circulation combined with any new foot wound, sore, or skin change.

How We Assess Your Heel Pain
A good treatment plan starts with an accurate diagnosis, and plantar fasciitis is a clinical diagnosis built primarily from your history and a hands-on exam rather than from a scan. When you visit either of our South Florida offices, we take the time to understand the full story.
We begin with a detailed history. We want to know when the pain started, where exactly it sits, what the first steps of the morning feel like, how your symptoms change across the day, your footwear, your work demands, and any recent changes in activity or training. These details often reveal the load pattern driving the problem.
From there we move into a physical exam and functional testing. We palpate the heel and arch to locate the tender point, typically at the inside edge of the heel bone, and we assess calf and ankle flexibility, foot posture and arch mechanics, and the strength of the foot and lower leg. We watch how you stand, walk, and move so we can see how the whole chain from the foot up through the hip is sharing the load.
Just as important is what we are ruling out. Plantar heel pain is an umbrella term, and several conditions can mimic plantar fasciitis, including heel fat pad irritation, a calcaneal stress fracture, nerve entrapment such as tarsal tunnel involvement of the tibial nerve, Achilles tendon problems, and inflammatory or systemic causes. Our differential approach is designed to make sure we are treating the right problem.
On imaging, our position follows the evidence. When imaging is warranted, we coordinate it and interpret the results within the bigger clinical picture.
Your Personalized Plantar Fasciitis Treatment Plan
There is no single fix that works for everyone, so we build a phased plan that meets your foot where it is today and progresses as you improve. Our approach is non-surgical and medication-free by design, and it reflects the same root-cause philosophy that guides everything we do at Carpe Diem Chiropractic.
Phase 1: Calm the irritation.
First, we reduce the daily strain that keeps the fascia angry. This usually means short-term load management and activity modification, guidance on supportive footwear, and simple at-home steps such as icing and gentle calf and plantar fascia stretching. Hands-on soft tissue work, including Active Release Technique (ART) and instrument-assisted soft tissue mobilization (IASTM), helps ease tension through the foot and calf, and kinesiotaping or supportive taping can offload the arch while symptoms settle.
Phase 2: Restore motion.
Once the heel is less reactive, we address the restrictions that contributed to the problem. Manual therapy directed at the foot, ankle, and calf, along with targeted stretch therapy for the gastrocnemius and soleus and mobility work for the ankle, restores the give the fascia needs. This phase is where many people feel the morning pain start to fade.
Phase 3: Rebuild capacity.
Lasting relief comes from making the tissue more resilient, not just more relaxed. We introduce progressive therapeutic exercise to strengthen the foot, calf, and lower leg so the fascia can handle real-world loads again. Strengthening the calf and intrinsic foot muscles is one of the most reliable ways to reduce the chance of the pain returning.
Phase 4: Return to activity and life.
Finally, we rebuild toward the things you actually want to do, whether that is walking the neighborhood, working a full shift on your feet, or getting back to running. We progress your loading, refine your gait and footwear, and give you a maintenance routine so the gains hold.
Indication-based adjunct technologies.
For some cases, especially stubborn or longer-lasting heel pain, we may add in-office technologies alongside the core plan.
- Shockwave therapy can be considered for chronic plantar fasciitis that has not responded to first-line care
- Laser therapy is sometimes used to support comfort and recovery.
These are tools, not shortcuts, and they work best layered on top of stretching, loading, and footwear changes.
A clinical note on modalities. Not every modality is right for every case. The reason we assess carefully before recommending anything is that the best plan depends on your specific findings, your goals, and how your foot responds over time. What the Research Says, in Plain Language
We believe you deserve to know where our recommendations come from, so here is a transparent summary of the current evidence.
- The leading clinical practice guideline (JOSPT, 2023) synthesized more than one hundred studies, including high-quality trials. It recommends manual therapy to the joints and soft tissues of the lower limb, together with plantar fascia-specific and calf stretching, for both short- and long-term pain relief and improved function. This applies to adults with classic plantar heel pain. The limitation worth remembering is that guidelines describe what tends to help on average and still require individualized judgment, since they cannot predict any single person’s response.
- A 2024 meta-analysis of shockwave therapy (14 randomized trials, 867 patients) found that shockwave reduced the thickness of the plantar fascia, although in that particular analysis the pain difference versus other non-surgical options did not reach statistical significance. Other reviews have reported meaningful pain and function gains, especially for chronic cases. This evidence applies mostly to people with persistent symptoms. The limitation is that results vary with device type, energy level, and protocol, so shockwave is best viewed as an add-on rather than a stand-alone cure.
- Reviews of the natural course of the condition show that plantar fasciitis is usually self-limiting, with roughly 80 to 90 percent of people improving within about twelve months and only a small minority, near five percent, ever needing surgery. This applies broadly to typical cases. The limitation is that most people improve does not mean quickly or effortlessly, and those with a higher body weight, standing occupations, or high running volume can take longer.
- An evidence-based review in American Family Physician concluded that supportive shoe inserts and stretching, particularly plantar fascia-specific stretching, are sensible first steps, and that no single treatment is strongly superior to the others. This applies to most uncomplicated cases. The limitation is that the underlying evidence for many individual treatments remains modest, which is why combining approaches makes sense.
- A network meta-analysis in the British Journal of Sports Medicine (2019) compared a wide range of treatment options and suggested that combinations of conservative care generally outperform any single treatment in the short term. This applies to plantar heel pain broadly. The limitation is that network analyses rely partly on indirect comparisons, so confidence in exact head-to-head rankings is limited.
The throughline across all of this is consistent with how we practice: stretching, gradual loading, hands-on care, and smart footwear form the foundation, with added technologies reserved for cases that need them.
Patient Case Study: From Sharp Heel Pain to Back on the Move
The following is a representative, de-identified example created to illustrate a typical care journey. It does not depict a specific individual.
Patient profile: A 47-year-old Broward County teacher who spends most of the workday on her feet, with about four months of right heel pain.
Baseline limitations: Sharp first-step pain every morning rated around 7 out of 10, difficulty getting through a full teaching day, and abandoned evening walks because of arch pain after standing.
Assessment findings: Tenderness at the inside of the heel bone, notably tight calves with limited ankle dorsiflexion, mild overpronation, and weakness in the calf and foot muscles. No red flags, so no imaging was needed at the outset.
Plan duration: A structured plan across roughly eight to ten weeks, with care tapering as function returned.
Interventions used. Early load management and footwear guidance, soft tissue work with ART and IASTM, supportive taping, calf and plantar fascia stretching, progressive therapeutic exercise to rebuild calf and foot strength, and gait refinement in the later phase.
Milestones:
- By week two, morning pain had eased to a manageable level on most days.
- By week four, she completed full workdays with markedly less discomfort and resumed short walks.
- By week eight and beyond, she was back to her regular evening walks with only occasional, minor symptoms and a maintenance routine to keep them away.
Outcome metrics: First-step pain reduced from about 7 out of 10 to 1 to 2 out of 10, standing tolerance restored to a full workday, return to her preferred walking routine, and notably higher confidence in her foot.
Maintenance plan: A brief daily stretch and strengthening routine, supportive footwear, and periodic check-ins, with guidance on managing future activity spikes before they become flare-ups.
